Services Include
Blood pressure assessment and ongoing monitoring
Medication review, reconciliation, and renewals
Chronic condition management
Evaluation and management of acute, non-emergent illness
Ordering, review, and interpretation of laboratory testing
Medical weight management, including GLP-1 medications when clinically appropriate
Patient education and care coordination
Evaluation and medical management of common mood and anxiety conditions
Acute Illness Visits
Evaluation of acute, non-emergent symptoms such as fever, respiratory illness, urinary symptoms, ear pain, sore throat, and gastrointestinal concerns.
Assistance with patient-provided at-home COVID or influenza tests may be offered when available. Laboratory testing is ordered through external laboratories when clinically appropriate.
